You will need this form if you are coordinating YWIH events. Make sure that you collect completed forms from each student attending an event, and keep copies on file for your records. The goal is to avoid any issues, but if/when they arise, we will have the piece of mind that the proper steps were taken in advance. Be sure you collect filled forms from each student, and keep the forms.
